Domain has exceeded the max defers and failures per hour

This is a feature on the server to help reduce account SPAM. When you receive this error it is a result of too many emails being sent to invalid email addresses. There are many reasons why you may receive this error:

Are you sending bulk email or sending email to your subscribers? Be sure you are sending email to a clean list of subscribers you have. Programs such as BriteVerify can help you with this.

One of your scripts has been compromised by a hacker and is being used to send SPAM. This is a common problem when your scripts are not constantly updated and secure. Be sure to update all your scripts and that the permissions on your scripts are correct.

One of your email account passwords may have been sniffed out and is being used to send SPAM. Try changing your email account passwords and scan your computer for malware. A great free program for doing this is MalwareBytes Free.

Do you have Boxtrapper enabled? If your SPAM Assassin score is too high and you receive a lot of SPAM, with Boxtrapper enabled all those senders are going to receive an email from your verifying themselves but the issue is, most are going to bounce resulting in a high sender failure rate on your account.
